Youth Who Excel Nomination Requirements:

• Age 14 to 20

All nomination must include:

• Completed Nomination Form (Accurately Completed).
•  The biographical sketch for the  youth nominee excellence should include a summary of the nominee’s demonstration of extraordinary leadership, and exhibition of quality participation in civic and community forums.
• Latest photograph of the nominee. Color photo head shot with light and plain background preferred. Minimum resolution must be 600 x 600 dpi in jpeg format.

Nominations received after the deadlines will not be considered by the award committee.
